Life! program Group Sessions
Life! is a free healthy lifestyle program helping you improve your eating habits, increase your physical activity and manage stress.
Funded by Victorian Government and managed by Diabetes Victoria.
Run by expert health professionals, the program is delivered as a group course or telephone health coaching service and shows eligible Victorians how to reduce their risk of developing type 2 diabetes, heart disease and stroke. (Please note, the Life! program is not available for people who are living with Diabetes.)
Eligibility (meeting one of below criteria):
A: Australia Diabetes Risk Score ≥ 12, and BMI ≥ 25 (BMI ≥ 23 for Aboriginal people and people who were born in Asia)
B: Cardiovascular Risk Score ≥ 10 (can be calculated by your GP)
C: Existing conditions (cardiovascular disease, gestational diabetes, moderate or severe kidney disease, prediabetes, familial hypercholesterolemia, serum choleterol > 7.5 mmol/L, systolic BP ≥ 180 mmHg or diastolic BP ≥ 110 mmHg, polycystic ovarian syndrome)
Diabetes Education
I provide diabetes education to people living with type 1 diabetes, type 2 diabetes and gestational diabetes under care plan. I work close with you and your significant ones, help you to develop an individualised diabetes management plan and to make informed decisions regarding your health. Through ongoing support and guidance, I will help you navigate the complexities of living with diabetes, thereby improving your quality of life and reducing the risk of complications associated with the disease.

complication screening
emotional health
travel and diabetes
driving and diabetes
hypoglycaemia management
hyperglycaemia management
sick day management plan
Insulin therapy.
